Pre-Single Session Counselling Questions (EXAMPLE)

Below is an example of the questions in the pre-single session counselling questionnaire.

The questionnaire can help you and your counsellor identify the specific problem or issue you’d like to focus on.

Questionnaire Example

  1. What is the one problem or concern that seems most important for you to focus on right now?


  1. In what ways is this problem/concern impacting on you and your life? Eg. Relationships, work, social life, health, etc.


  1. How long has the problem/concern been in your life?


  1. What have you tried in the past that has been helpful for this situation?


  1. What have you tried that has been less helpful or even made the problem/concern worse?


  1. What would you like to get out of the session?


  1. What do you think is the most important thing for the counsellor to understand about the background of the problem/concern in your life?


  1. What personal strengths do you have that will help you in dealing with the problem/concern?


  1. What would those close to you say in answer to the above question?


  1. In order that our session is as helpful as possible, is there anything else that you think it is important for the counsellor to know?
