The questionnaire can help you and your counsellor identify the specific problem or issue you’d like to focus on.
Questionnaire Example
- What is the one problem or concern that seems most important for you to focus on right now?
- In what ways is this problem/concern impacting on you and your life? Eg. Relationships, work, social life, health, etc.
- How long has the problem/concern been in your life?
- What have you tried in the past that has been helpful for this situation?
- What have you tried that has been less helpful or even made the problem/concern worse?
- What would you like to get out of the session?
- What do you think is the most important thing for the counsellor to understand about the background of the problem/concern in your life?
- What personal strengths do you have that will help you in dealing with the problem/concern?
- What would those close to you say in answer to the above question?
- In order that our session is as helpful as possible, is there anything else that you think it is important for the counsellor to know?
