Covid-19 Conditions of Entry


Mask Policy:

  • Surgical masks are required for face-to-face medical appointments such as with a doctor or nurse (masks can be provided by FWHC at Reception).

  • Wearing a mask upon entry, at Reception and for Counselling/Group Sessions is optional.

  • Please call Reception on 9431 0500 if you have any queries.


If you answer YES to any of the following questions, call us on 9431 0500 prior to your visit.

  1. Do you currently have Covid-19?­

  2. Do you have any of the following symptoms:- respiratory illness
    – cold or flu
    – fever
    – cough
    – sore throat
    – shortness of breath
    – aches & pains
    – headache
    – diarrhoea
    and/or
    – vomiting?

  3.  Are you waiting for the result of a recent test for Covid-19?

  4. Have you been a household contact or had a close personal interaction with a person with COVID-19?

If you answered NO to all the above questions, the following conditions apply:

  • All clients to wash or sanitise hands upon entry.

  • Surgical masks will be required for face-to-face medical appointments (masks can be provided by FWHC at Reception).

  • If you are unwilling or unable to wear a mask, a telephone appointment may be arranged.

  • Please call Reception on 9431 0500 if you have any queries.
